Please do remember to submit incident reports to CORESS, which you can do in confidence online at coress.org.uk. Reporting to CORESS ensures that the lessons you have learned from incidents in your own theatre, are shared with the wider surgical community, to prevent similar mishaps in the future. Incidents may be diagnostic or operative errors, technical or maintenance failures, regulatory or procedural issues, or unsafe practices or protocols. All reporters to CORESS receive a certificate of contribution, which can be used to demonstrate continuing professional development for revalidation purposes.
